- Make sure you have heard and clearly understood your doctor's/ nurse's instructions
- Make sure you get the results of any test or procedure. Ask your doctor or nurse details on the test results, and understand what the report indicates
Preventing medication issues for outpatients
Several medication safety checkpoints have been set up by the hospital. However, make sure that the five R's (the right patient, right drug, right dose, right time and right route of administration) have been adhered to:
- Make sure that your care providers are fully aware of the medication you are taking. This includes prescription, over the counter medicines and dietary supplements like vitamins and herbs.
- Ensure that the doctors, nurses and pharmacists know about any allergies that you have experienced or any side effects that you may have had to medication.
- Find out and understand more about your medicines. Question anything that doesn't seem right to you:
o What is this medicine for?
o What is the dosage?
o What are the side effects?
o When do I take the next medications?
- If you have doubts about the medication and are uncomfortable receiving it, do not take it. However, make sure you discuss it with your doctor at the earliest.
- If you would like to speak to a pharmacist about your medication, please ask your doctor or nurse to contact one for you.
